CsvMail is an HTML form mailer and manager. It mails the results of form submissions and also writes the entry to a secure.csv file. It requires minimal HTML form changes - just change the beginning form tag to post to CsvMail. It is very easy to configure, but includes plenty of extra options, including domain security settings and a bounceback required fields option. A fully-functional evaluation version is available.
The Cybercafe Organizer (CybOrg) is a point of sales and administration system for Internet cafes (cyber cafes). It has a Web-based interface and is written in Perl using Template Toolkit and a RDBMS. Forms for Metadot Web Portal is a web based forms package for the metadot portal. From your browser you can define a form with checkboxes, radio buttons, etc. Validate minimum length, max size, or against a regexp, attach "help" to field, and usual forms package type stuff. Users can then use the web to enter data according to your forms. It works within the metadot portal environment (nice, flexible portal - gpl mod_perl mysql).

Office Tournament is a tool for keeping track of office tournaments, such as foosball. You may enter data from the command line or from the CGI Web interface. It outputs all kinds of stats and even a nice chart if you use the Imager::Graph::Pie Perl library available from CPAN. It currently requires no database to run, but there are plans to migrate the text (flat file db) to MySQL.
