Polliz

Polliz is a script for managing online opinion polls. It's very simple to use and configure, and uses only one file and a MySQL database. It features administration for user management, which can be used to give different levels of access to administrators of large Web sites. For instance, some users can add new polls, others can decide if a poll should be put online or not, and others can only see which polls are active.
