phpFormGenerator

phpFormGenerator is an easy-to-use tool to create reliable and efficient web forms. No programming of any sort is required. It has the ability to create up to 100 form fields, and to add a variety of field types including text boxes, drop down selection menus, check boxes, radio buttons, and freeform text areas. All field types are highly customizable. The form data can be sent to a provided email address, or stored in a database table (currently MySQL).

Release Notes: This release includes a small feature enhancement in which the admin portal now comes with a "print record" functionality that lets you view a printable version of your form entries.

Release Notes: Some users complained that they could not delete the forms created by the phpFormGenerator. For that reason, this is an updated version of 2.09 that includes a feature to delete old forms. Also, a minor bug in with the link to the admin portal has also been fixed.

Release Notes: This version makes it easy to generate multiple forms, by prompting for a project name for every form created.

Release Notes: In this release, file upload fields are no longer "required" fields. This release also supports templates: currrently there are two available.

The Fantastico version has no problems, it just depends on the ISP's configuration of the server and the installation procedure. However, I do recommend that users should just do a manual installation of the phpFormGenerator to avoid any problems that might occur during automated installations (such as in Fantastico).

Secondly, providing technical support is not an obligation. I expect users to first check and post on the project's forums on Source forge if they have any problems or questions. The forums are very active and answers are provided by users (and myself). However, if you email me directly regarding a problem I may or may not answer, depending on my availability, the type of question you asked, etc. Generally, I simply ignore questions that have been asked thousands of times by other users and to which the solutions are all over the forums.

Finally, phpFormGenerator has thousands of users who have successfuly installed and are extensively using the program. This should give you a hint that there might be something wrong in the way you are approaching things.

But know this...
As of this writing:

The Fantastico version has problems (according to the developer's site - and I could not get consistent file retrieval from sender uploads)

The demo version doesn't work.

The developer didn't respond to any emails I left (over about a week) for the for-fee support he offers (to his gmail addy, the .it one bounces) when I asked for help with a fresh install after having trouble with the new (non-Fantastico) script.

phpFormGenerator for the masses
phpFormGenerator is an excellent tool to be distributed by ISPs providing web hosting services to general webmasters. It lets you create web forms very easily, without an programming effort and also gives you the ability to link it with a MySQL database. The transaction on the database side is also handled by the program, you don't even have to create the db table before hand! The simplicity and ease of use combined with the clean and imageless interface makes it a killer system.
